The sinuses are air-filled cavities behind the forehead, cheeks and eyes, lined with mucus membrane that drains into the nose. Ayurveda classified nasal disease with real precision. Pratishyaya — disorders of the nose — is described in five types by dosha, and when an untreated or wrongly managed Pratishyaya becomes chronic and repeatedly recurring, it becomes Dushta Pratishyaya (“spoiled” or vitiated Pratishyaya) — the classical picture of chronic rhinosinusitis. Sinusitis is also correlated with Kaphaja Siro Roga (a Kapha disorder of the head) for its heaviness and headache.

The mechanism the classics describe maps cleanly onto the modern one: the nose is the gateway to the head, and the head is the principal seat of Kapha. Aggravated Kapha accumulates as mucus in the sinuses and nasal passages; disturbed Prana Vata (the sub-dosha of the respiratory tract and head) blocks the drainage and drives the sneezing reflex; and weak Agni produces Ama that thickens the secretions and, with depleted immunity, keeps the passages hypersensitive. This is precisely why a nasal spray alone never resolves it — it addresses none of these.

Why nasal sprays and antihistamines alone are insufficient: A steroid spray shrinks the lining and an antihistamine blocks the reaction — giving relief while used, and relapse when stopped. Neither clears the accumulated Kapha, opens the blocked drainage, digests the Ama or rebuilds the immunity keeping the nose hypersensitive. Repeated antibiotics for recurrent sinusitis further weaken Agni and immunity.
